Hospice Care is a difficult time for patients and their families. We understand the need for the patient to be comfortable and to have their dignity, and to allow the family to grieve however way they want to. And that’s is why our dedicated and compassionate caregivers are here to help.

Hospice care and Hospice Home Care help hospice patient to be comfortable and continue to maintain their dignity during their last days. Compassionate Caregivers provide hygiene services like bathing, toileting and other activities of daily living.

Who is Eligible for Hospice Care

Anybody that has been diagnosed with a terminal illness

• Prescription Medication related to the hospice diagnosis

• Durable Medical equipment

Who Provides Hospice Care

Care is provided by our partner of "Interdisciplinary Team" of professionals; your physician, our partners of Medical Directors, Nurses, Home Health Aides, Social Worker Chaplains, Dietitian, Wound Care Nurse Specialist, and Volunteers. This team works together following an individualized "Plan of Care".
